Why add Hazard Assessments to forms?
When employees are filling out forms, having hazard assessments readily available helps them understand the specific hazards they might encounter. This ensures that they are aware of potential risks as they perform their tasks, leading to better decision-making in real-time and preparing them for any immediate actions.
How do I add Hazard Assessments to form templates?
To prepare, you will need to have at least one published Hazard Assessment. You will need to have the permission to edit any form (Inspections, Meetings, Incidents) templates.
Instructions:
1. Click on any of the forms modules (Inspections, Meetings, Incidents).
2. Click the forms template library
3. On the template that you would like to add the Hazard Assessment to, click on the edit icon next to the template name. You will then be taken to the template editing page
4. You will be able to see the new question type when either editing an existing form template or creating a new one. You can also make the Hazard Assessment question mandatory.
5. Save the template and make sure it is published.
Once hazard assessment question type is added in the template, users can start using it when creating a new form. The new question added will not apply to existing forms.
How will this work in the form workflow?
When creating a new form, employees can now select the hazard assessment and the associated tasks to review then acknowledge reviewing the hazard assessment directly in the form.

What happens when I can't see a list of Hazard Assessments or a specific Hazard Assessment in the form template?
A Hazard Assessment will not appear in the available list unless it is published. To publish a Hazard Assessment, click the Hazard Assessments module, then click the 'Publish' button next to the Hazard Assessment name you need to add to your form template.
