Why are there no questions listed when starting or editing an Interview/Observation/Documentation Review ?

When starting or editing an interview/observation/documentation review, there are no questions listed although you can enter Title and choose Interview Type, Role and Site/Location etc.

This is caused by no audit elements selected for the audit.

Resolve this issue by going to "Select Audit Content" on the left tree (right above Process Flow) then select Yes for at least "Include Certificate of Recognition (COR)?".

(The other questions are for optional elements) 

You will then see the questions showing up for interview/observation/documentation review.
