What is this feature?
Continuing the enhancement of eCompliance forms, safety managers now have the ability to add logic to form templates (incidents, inspections, or meetings), enabling forms to be dynamic. Logic can be applied to pass-fail and single-select question types on non-scored forms. Below is the logic that can be added to a question when a specific answer is selected by a user:
-
Mandate corrective actions
-
Mandate an image attachment
-
Hide specific form sections
-
Ask follow up questions
What is included in this feature?
Safety managers can add, customize, and remove form logic (only on pass-fail and single-select question types on non-scored forms)
Based on the form logic rules front-line workers can:
-
Answer questions that contain form logic
-
Attach mandatory images when required
-
Create mandatory action items when required
-
Select an answer which triggers a section or multiple sections to be hidden
-
Answer follow-up questions
Which users can add form logic to forms?
Any manager that has the ability to edit a form template can enable form logic on forms.
Does this apply to all 3 form modules?
Yes, this feature will work on Inspections, Incidents, and Meetings.
Does this feature work with Hazard Assessments?
No, this feature is specific to the forms modules.
Will this work with scored forms?
Version 1 is only for non-scored forms. We will get feedback from customers before we enable this for scored forms.
Which question types can form logic be applied to?
Logic can only be added to the following question types:
-
Pass/Fail/NA
-
Single-select question types
How much does this feature cost?
There is no additional fee to this feature.
What happens to our existing templates once this feature is enabled for our account?
Upon release safety managers will have the ability to add logic to their existing templates (by editing them) or new templates. However form logic will only be added if they do so (we are not automatically adding any logic to their forms)
Can this feature trigger actions in other form templates?
No, this scope is limited to actions and responses on the form template that is being modified. So if a safety manager is adding form logic to TEMPLATE A, then the form logic will only be applied to TEMPLATE A. Not any other template in any other form module.
Are there any additional changes to the forms modules with this enhancement?
Besides the additional functionality stated here all other features for the forms modules will continue to work as intended, including the permissions, site, and distribution rules for forms.
How does this affect Safety Intelligence?
Follow-up questions as well as created action items will be captured in Safety Intelligence.

Adding form logic to a form
What are the best practices for adding form logic?
Before adding form logic to a form we recommend that safety managers create all of the sections and questions for their form first, and then add the logic afterwords.
How does a safety manager add logic to questions?
Under pass-fail and single-select questions on non-scored forms, there will be a button labeled “Add Logic”. Once clicked, at the bottom of the question there will appear two drop-downs.
-
For “If answer is” - select which answer in the question will have logic
-
For “then” - select the logic that will be added (make photo mandatory, make action item mandatory, ask follow-up question, hide sections)
Can I add smart form logic to forms that were made before the feature release?
Yes you can! You can add form logic to existing forms when edit the form template.
Can there be multiple types of logic on the same question?
Yes, there can be multiple types of logic on the same question. Managers are able to add logic for every answer and combine logic for the same answer.
Can N/A contain logic if it is enabled as an answer to a question?
Yes, N/A will show as an option in form logic.
Will answers in hidden sections get deleted?
Answers in hidden sections will only be deleted once the form has been submitted for sign off. Before submission, if a user un-hides a section as a result of form logic, the answers answered before it was hidden would re-populate.
Make a photo mandatory
Can a photo be made mandatory to attach to a question regardless of what is answered?
Yes, to achieve this add the “make photo mandatory” logic for each answer in the question.
Can the file type of the photo be specified?
No, the file type cannot be specified. Users can attach any image file type mentioned in this support article.
What happens if a manager decides to make a photo mandatory multiple times for the same answer to a question?
In this case when completing the form the user will only have to attach one photo to meet this requirement. So even if a manager sets at the template that for a specific answer a photo is mandated 5 times, the user completing the form only has to submit the photo once.
This will be modified post the beta to limit managers from adding this specific requirement more than once so that the behaviour is clear.
Make an action item mandatory
What will happen if a user does not have action item permissions enabled and they trigger a mandatory action item?
The user will be presented with an error message instructing the user to contact their administrator to enable their action item permissions. The user will not be able to submit the form due to the mandatory action item until permissions are granted.
What happens if a manager decides to make an action item mandatory multiple times for the same answer to a question?
In this case when completing the form the user will only have to create one action item to meet this requirement. So even if a manager sets at the template that for a specific answer an action item is mandated 5 times, the user completing the form only has to create one action item.
This will be modified post the beta to limit managers from adding this specific requirement more than once so that the behaviour is clear.
What happens to a user who is required to create an action item because it is mandatory as per form logic however they do not have this permission?
In this scenario, this specific user will be able to submit the form without creating an action item. This is because they do not have the permission to do so.
Ask follow-up questions
How many follow-up questions can be added?
Safety managers can only ask up to 5 follow-up questions per answer.
Can logic be applied to follow-up questions?
No, follow-up questions cannot contain logic.
What question types can be added as a follow-up question?
Any question type can be added as a follow-up question. The follow-up questions can also be toggled to mandatory (do note that they will only be mandatory to answer if the user completing the form triggers the follow-up question).
Will follow up questions be displayed on the excel export?
Yes, follow up questions will be displayed on the excel exported sheet.
Will follow up questions be displayed on the PDF as well?
Yes, follow up questions will be displayed on the generated PDF.
Hide sections
What will happen to mandatory questions in sections that are hidden by form logic?
Users will not be required to complete the mandatory questions that are within sections hidden in the form due to form logic.
Is there a limitation on which sections on the form can be hidden?
No, all sections except the section the question is in can be selected to be hidden. Multiple sections can be hidden as well.
PDF export
What changes are being made to the form PDF?
For forms that have logic applied, the PDF will have the following changes
-
Follow up questions that are answered due to form logic will be displayed on the form pdf.
-
When a user selects filtered report then any sections that are hidden due to form logic (of the user's provided answer) will not display on the form PDF.
Mobile app
Can safety managers add form logic on the mobile app?
No, they can not. Form templates can only be edited on web.
Safety managers can however edit form templates on responsive web (logging into eCompliance from their mobile browser).
What is the minimum mobile version that is needed to use this feature
Mobile version 7.11
Download the mobile app here: Android | iOS
Are mobile users required to download the latest version of the app to use this feature?
Once form logic has been added to a form template and published, only the users who have access to a form that has form logic will be required to update their app once their forms have synced.
How will this work on the mobile app (and tablet devices)
The experience on web and mobile are the exact same
-
Mandatory photo attachment - if a mandatory photo is required the user can not submit the form until they attach the image to the question (upon submission the form will scroll the user to the first missed attachment and display an error message and icon)
-
Mandatory Action item - if an action item is required the user can not submit the form until they create an action item for the question (upon submission the form will scroll the user to the question that requires the action item and display an error message and icon)
-
Hide sections - based on the users answer the entire section will be hidden from the form
-
Ask a follow up question - based on the user’s answer they will be presented with follow-up questions
